Job Description:
A company is looking for a Case Manager RN (Remote).
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
for members
Develop proactive care plans to improve member wellness and
outcomes
Utilize clinical judgment and data to evaluate member needs and
address barriers to care
Required Qualifications and Education
Active and unrestricted RN licensure in Georgia or compact RN
licensure
3+ years of clinical experience
2+ years of Case Management, Discharge Planning, or Home Healthcare
Coordination experience
Bachelor's degree or associate degree with equivalent
experience
Certified Case Manager preferred
